Trainee/Assistant Practitioner

Job summary

This is an exciting opportunity to join our well established Community Services in a brand new role. We are looking for 2 dynamic and motivated individuals to join us by working in an integrated way across multiple community services. We provide healthcare and support for patients in their own homes to offer a safe and effective alternative to hospital admission, and teams that are proactively keeping people well.

It is a community role where you will see patients in their own homes, using your own car for transportation. You will help coordinate the patients care and be an integral part of their care provision.

For the right candidate a training role commencing on B3 can be considered to complete Level 5 Assistant Practitioner Diploma and progress to band 4 on successful completion of this program.
