As HomePlace’s Transition Coordinator, you will facilitate smooth, person‑centred transitions for people living with disability who seek to have their disability support from HomePlace.

As HomePlace’s Transition Coordinator, you will facilitate smooth, person‑centred transitions for people living with disability who seek to have their disability support from HomePlace. This includes coordinating all aspects of the transition process, managing enquiries, liaising with stakeholders, and ensuring each participant’s transition is respectful, well‑planned, aligned with their NDIS plan and goals, and is a positive experience for them and their support network.

Your roles and responsibilities will include (but are not limited to):
