Job Description
The Transitional Care Pharmacist plays a vital role in the multidisciplinary team at Hendrick Medical Center, ensuring a smooth and safe transition for patients between inpatient and outpatient care.
Key responsibilities include:

Collecting accurate medication histories from patients and reliable sources upon admission.

Ensuring hospital providers have complete medication information for optimal treatment.

Monitoring patients throughout their hospital stay to support discharge planning.

Coordinating with outpatient pharmacy services to fill discharge medications.

Assisting patients with medication access issues, including financial barriers.

Collaborating with providers to confirm all necessary discharge prescriptions.

Providing patient education on medication regimens before discharge.

Opportunities to work across various hospital departments and specialty outpatient clinics.
Job Requirements
Education:

Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD)
Experience:

No prior experience required
Licenses/Certifications:

Active license as a Registered Pharmacist in the State of Texas
