Job Summary

The Transitional Care Pharmacist plays a vital role in the multidisciplinary team at Hendrick Medical Center, ensuring a smooth and safe transition for patients between inpatient and outpatient care.



Key responsibilities include:





  • Collecting accurate medication histories from patients and reliable sources upon admission.




  • Ensuring hospital providers have complete medication information for optimal treatment.




  • Monitoring patients throughout their hospital stay to support discharge planning.




  • Coordinating with outpatient pharmacy services to fill discharge medications.




  • Assisting patients with medication access issues, including financial barriers.




  • Collaborating with providers to confirm all necessary discharge prescriptions.




  • Providing patient education on medication regimens before discharge.




  • Opportunities to work across various hospital departments and specialty outpatient clinics.






Job Requirements

Education:





  • Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD)





Experience:





  • No prior experience required





Licenses/Certifications:





  • Active license as a Registered Pharmacist in the State of Texas
