Join Our Dynamic Team as a Case Manager Job Description: - Start Date: ASAP - Specialty: Registered Nurse, Case Manager - Shift: Day 5x8-Hour (08:00 - 16:30) - Patient Ratios: 1:20 - Acute Care Experience Required: 2 years of acute care hospital case management focused on care coordination and discharge planning - Weekend Requirement: Every other weekend coverage to meet departmental needs - California State License: Required (pending licenses accepted if licensed upon start) - Pre-Approved Time Off: 1 request allowed - Orientation Hours: 40 hours Essential Skills: - Care Coordination and Discharge Planning - Knowledge of InterQual Criteria* - Experience with CMS: Centers for Medicare and Medicaid Services - Understanding of DRG (Diagnosis Related Groups)* - Ability to assess patients and organize family meetings - Experience with HMOs, IPAs, and arranging DME, SNF, and ambulance transport - Proficiency in Microsoft applications Preferred Experience: - Working with patients experiencing drug/alcohol abuse, homelessness, and mental health crises - Development of comprehensive treatment plans - Interdisciplinary support collaboration, including social services and transportation Why Catalytic Solutions (CatSol): At CatSol, we connect dedicated therapists with rewarding assignments across the country.
