Occupational Therapist Location: Haxtun, CO Agency: Mindlance Health Pay: $2,166 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/16/2026 About the Position ID: 75315629 Shift: 8hr Days 8x Description: HWL Main Street Clinic-NANC-0017 OT needed at Haxtun!\r\n8 or 13 week contract, flexible for 4-10's or 5-8's. Department is open from 7a-6pm. Guarantee 36 hours!\r\ninpatient/swing, outpatient, extended care unit. Min 1 yrs OT experience required , CO license required ( can be pending at time of submission). EMR is Athena.\r\nWe have 5 inpatient/swing beds, 20 extended care beds and an OP clinic.\r\nHWL prescreen with manager final interview and offer\r\nMinimum Qualifications:\r\nBachelors or masters degree in occupational therapy from an American Occupational Therapy Association (AOTA) or the Accreditation Council for Occupational Therapy Education (AOTE) certified school.\r\nCurrent license as an Occupational Therapist Registered in the State of Colorado.\r\nBasic Life Support (BLS).
