Exciting Opportunity for a Case Manager Job Description: - Position: Registered Nurse - Case Manager - Start Date: ASAP - Shift: Day 5x8-Hour (08:00 - 16:30) - Patient Ratios: 1:25 - Weekend Coverage: Every Other Weekend - Years of Experience Required: 2 years of acute medical Care Manager experience in Care Coordination - Travel Experience Required: Yes (HARDSTOP - must have travel experience) - Patient Age Groups: Adults, Geriatrics - Daily Census: Up to 25 patients Responsibilities: - Conduct thorough patient assessments for effective care coordination. - Develop and manage personalized discharge planning for patients. - Organize and facilitate family meetings to ensure comprehensive care. - Collaborate in the development of treatment plans tailored to patient needs. - Remain knowledgeable about adoption/foster care, mental health support, and other specialized care options. - Independently arrange DME, SNF, and ambulance transport as needed. Required Skills: - Proficient in computerized documentation (Epic required). - Understanding of regulatory guidelines (CMS, HIPAA, OSHA). - Experience in case management settings, including acute hospitals. Preferred Skills: - Experience in Emergency Department, ICU, Medical/Surgical, and Surgical Services. - Familiarity with HMOs, IPAs, and care for patients facing substance abuse or homelessness. Special Equipment: - Competent with required computerized documentation.
