Overall Job Responsibility

In this role, you will develop holistic care plans through comprehensive bio-psycho-social assessments of families. You will provide case management support to safeguard the long-term welfare and financial security of persons with disabilities, including facilitating Future Care Planning (FCP) and setting up of Special Needs Trust (SNT) where relevant, and conducting regular reviews of care plans for SNT beneficiaries.

Duties and Responsibilities

• Engage families in FCP conversations and help them plan for persons with disabilities' long term care needs.
• Assess eligibility of clients for sponsorship schemes and facilitate applications.
• Assess eligibility and suitability of the SNT for the client and facilitate Trust setup accordingly.
• Annual Trust review, formulation and implementation of plans to earmark assets to the Trust.
