Urgent Care Response General Practitioner

Are you looking for anexciting role providing medical oversight for community multi-disciplinaryteams working to prevent unnecessary hospital admissions for people whoseneeds are best met in their own home?

We are looking forregular experienced GPs to join our integrated team working flexibly on aMonday and/or Tuesday. The hoursavailable include 08:00-20:00 or 08:00-14:00 and 14:00 20:00.

The role will be basedat Nelson House in Hereford.

This role wouldideally suit GPs who would welcome the time to unpick complex medicalhistories and support proactive care planning; those with an interest infrailty; and those who enjoy supporting the learning and development ofadvanced practice colleagues.

Salary :- In hours rate from £46.12ph - 47.75ph,Otr of Hours rate ( 6pm-8pm & weekends) £55.34ph - £57.65ph.
